(August 18, 2016) — Zachary Smith was scratching uncomfortably when his mom, Mandy picked him up at school. He claimed his feet and even his eyes itched. He and his mom assumed the rash was poison ivy, and put some anti-itch cream on it. That was not the end of it. Dermatologists couldn’t have expected the awful experience that lay in wait for Zachary Smith, a small, brown-haired boy. The next day Zachary’s rash covered his body and his temperature soared.

In a panic, Mandy rushed her son to the hospital. Doctors were puzzled and very concerned. They tried medications for a viral infection, but the medicines made no impression. Zachary’s eyes became bloodshot, and his lips cracked and bled. His condition was rapidly becoming critical.

A nurse’s frightening suspicion was soon verified: The child had Stevens-Johnson Syndrome (SJS), a rare and potentially fatal disease. SJS attacks skin and mucous membranes. Its ulcerative rash and flu-like symptoms rapidly become deadly.

SJS is a toxic condition in which cells die and the epidermis, the outer skin layer, pulls away from the dermis, or inner layer. Ulcers and lesions form in the mouth, eyes, genital and anal regions. Conjunctivitis, or eye inflammation, occurs in 30 percent of children affected. SJS is considered a reaction by an immune system gone awry. The reaction can be triggered by viral infections, cancer or medication. The most common trigger is medication.

Zachary’s mom confirmed medication as the culprit. He had been taking a medication called lamotrigine (lamictal) for migraines. She had given her son medicine for a migraine just days earlier.

Zachary’s skin began to pull away from his body. Even the lining of his lungs began to loosen. It literally burns you from the inside out. Doctors put him in an artificial coma and removed the dying top layer of his skin. It eventually effected 90% of Zachary's dermal layer. A wrapping of artificial skin then took the place of his epidermis. This would protect him from further infection if he survived his terrible ordeal.

Goddamn I remember when I got this I got lucky though this poor kid bless his soul the pain is horrible. I got treated like a burn victim all of the skin on the bottoms of my feet fell off like water logged skin and all the skin in my mouth inside cheeks sloughed off even under my tongue. It was affecting all of the moist areas of my body. It had just started to move onto my arms and other dryer areas when I started to get better. I got lucky so lucky it took me a year before I could walk without pain. Never forget it and mine by the way was triggered by Ibuprofen so think on that lol. Won't take it at all for anything now

Antiepileptic and anti-infectious agents were the most common drugs associated with Stevens-Johnson syndrome (SJS)/toxic epidermal necrolysis (TEN) in children in an analysis of a World Health Organization (WHO) database.

METHODOLOGY:
SJS and TEN are rare, life-threatening mucocutaneous reactions mainly associated with medications, but large pharmacovigilance studies of drugs associated with SJS-TEN in the pediatric population are still lacking.

Using the WHO's pharmacovigilance database (VigiBase) containing individual case safety reports from January 1967 to July 2022, researchers identified 7342 adverse drug reaction reports of SJS-TEN in children (younger than 18 years; median age, 9 years) in all six continents.

Median onset was 5 days, and 3.2% were fatal.

They analyzed drugs reported as suspected treatments, and for each molecule, they performed a case–non-case study to assess a potential pharmacovigilance signal by computing the information component (IC).

A positive IC value suggested more frequent reporting of a specific drug-adverse reaction pair. A positive IC025, a traditional threshold for statistical signal detection, is suggestive of a potential pharmacovigilance signal.

Overall, 165 drugs were associated with a diagnosis of SJS-TEN; antiepileptic and anti-infectious drugs were the most common drug classes represented.

The five most frequently reported drugs were carbamazepine (11.7%), lamotrigine (10.6%), sulfamethoxazole-trimethoprim (9%), acetaminophen (8.4%), and phenytoin (6.6%).

The five drugs with the highest IC025 were lamotrigine, carbamazepine, phenobarbital, phenytoin, and nimesulide.

All antiepileptics, many antibiotic families, dapsone, antiretroviral drugs, some antifungal drugs, and nonsteroidal anti-inflammatory drugs were identified in reports, with penicillins the most frequently reported antibiotic family and sulfonamides having the strongest pharmacovigilance signal.

Vaccines were not associated with significant signals.

The study provides an update on "the spectrum of drugs potentially associated with SJS-TEN in the pediatric population," the authors concluded, and "underlines the importance of reporting to pharmacovigilance the suspicion of this severe side effect of drugs with the most precise and detailed clinical description possible."

It is a very very dangerous thing to go through it's harder on the little babes like that child. A lot of people don't survive it
