Staged Phalloplasty: Female-to-Male Surgery with an Abdominal Flap
Phalloplasty is surgery that creates a penis, including as part of gender-affirming care or reconstruction after injury or for congenital anomalies. It usually involves three to four stages, with the number depending largely on urethral reconstruction. Tissue may come from the forearm, thigh, abdomen or back. Abdominal pedicled flaps do not readily support a tube-within-a-tube design, making them less suitable for urethral lengthening. An erectile implant is optional. Recovery typically includes at least a week in hospital and a urinary catheter during healing. Staged care can take 12 to 18 months, and full function usually takes a year or more. A systematic review reported urinary fistulas in 34.1% of cases, urethral strictures in 25.4%, partial flap loss in about 11% and total flap loss in about 3%. These figures cover phalloplasty broadly, not abdominal flaps alone. The first recorded successful phalloplasty was performed by Bogoraz in 1936 using a tubed abdominal flap after traumatic amputation. |
