It takes so long because it is several layers deep, past tendon etc, and something that deep is very difficult to heal. That is why bed sores are so awful for patients to suffer with.
I'm not sure what you mean by sugar but I assume you mean blood sugar as in diabetes? If so, this wound is one caused by trauma and made worse with poor circulation caused by smoking. It is not caused by the circulation of a diabetic.

I broke my ankle and still smoked, thank god mine didn't do this. I would've stopped smoking too!
