Serious Rash - Eczema Herpeticum
A 16-year-old boy presented with 3 days of fever and painful rash. There was a history of dermatitis without a formal diagnosis, treated with nonsteroid-based moisturizing lotions. The patient's mother had chronic eczema. Physical examination revealed a temperature of (38°7C). Heart, lung, and abdominal examination results were normal. A rash as depicted in the photographs (Figure 1, Figure 2, Figure 3) was present over the torso and extremities, sparing the face and mucous membranes. The patient was admitted to the hospital. Diagnosis Eczema herpeticum A presumptive diagnosis of eczema herpeticum (Kaposi varicelliform eruption) was made at admission. The key features were the diffuse nature of the rash and the characteristic umbilicated lesions. The patient was admitted for intravenous acyclovir and vancomycin. Viral culture of a lesion grew herpes simplex 1. Direct fluorescent antibody of lesional fluid was positive for herpes simplex and negative for varicella. Eczema herpeticum is an uncommon complication of atopic dermatitis and other skin disorders because of impetiginization of abnormal and normal skin with herpes simplex. Mortality in the preacyclovir era has been reported as high as 9%. My daughter has eczema as well (without the herpes) but not nearly as bad as this.  |

OMG..that is awful. When Jake was less than a year old, he had Eczema around his mouth, a small pink area about the size of a dime. It was gone by the time he turned one and has never returned. It's fairly common in babies. This poor guys back is a mess.  |

My youngest niece had it bad when she was a baby, she was a chubby little tub, always in the crooks of her elbows and around her little ankles. Poor sweetie, sometimes she wouldn't even let anyone hold her.

Jake's was (I think) from all the drooling...seems like the location was perfect for it. My best guess anyway. It would be awful to have it like your little niece did, can't imagine that discomfort. Our friends daughter had it like that, and that poor thing was in constant discomfort.  |

Yeah, my brother and sis-in-law tried all the creams and lotions and oils they could, it just had to go away as she grew up.  |

I get eczema every year, but NEVER that bad. Still getting over a nasty case that I had under my cast. Blech!
