That guy is wearing clothes so he went to that clinic/hospital that morning! so that rules out the use of medical-grade maggots with the aim of removal of dead tissue. So he's injuried his little fella and a fly has lain eggs on his when he's slept nude. It's not lack of cleanliness as a smelly dick (bacteria) doesn't breed maggots... although it may attract their parents. |
