The baby doesn't look full-term. I don't think she gave birth, per say. Perhaps she miscarried, or delivered premature or stillborn. She looks like resignation, pain or hurt (emotional and physical with all that blood loss). And those people, especially with the name badge is coming to help her.

It's extremely rare I call something fake, and even rarer I go to lengths to comment that it is fake, but look at the texture of the "blood"...I've seen enough real blood to know that is very obviously ketchup.

Wow, you have a great eye! I did a quick search and came up with an article about it being a mock car accident - there was more that included needed a helicopter, also. I couldn't get to the article, it was only available to subscribers. But from what I could see it was for educational purposes in Arkansas.
