The first, third and fourth pics are real - you can see the swelling. They use pumps with a small barrel which suck up the area and hold it there for a long time, the blood pulls up into the area and makes it swell. It's kind of the smaller version of the "penis" pump.

Alot of women use these during breast feeding if their kids won't latch on right, gives them a slightly larger area to work with, so to speak...
