This looks like an electrical burn.
In some cases (usually severed fingers) doctors "plant" one body part into another, in order to feed it and not allow it to die. In this case they apparently attached the index finger to a strip of healthy tissue from the middle finger to "feed" the index.
I'm not a doctor, but they did something similar to a close friend of mine a couple years ago.
