Has he been on any steroids? Sometimes they can mitigate some of the symptoms.Of course long term use has its own set of side effects.

I’ve had this for over 20 years. Sometimes had deep fissures in the tongue. Then is suddenly went away. Haven’t had it for a few years now.

i believe these lesions exist on his tongue becuz he had a severe case of thrush as a child. the doctors tried everything possible but it was very persistant, despite my boiling bottles & nipples between every use & very diligent hygiene. i don't know this is the case i only suspect. may possibly have been the meds that were used themselves. at any rate, the doctors say nothing can be done. he is 14 and still cannot eat or drink anything hot or even mildly spicy. even orange juice is uncomfortable.
The tongue needs to be quarterized, sealed with a laser, ie burn seal it then heal it. It needs to be done progressively, bit by bit till it's all done as it will just keep splitting throughout his life.

Doesn’t look like geographic tongue, because I’ve had it for decades. GT presents as patches across the surface of the tongue, normally in a shape of a continent, along with fissures in the surface.

This boy’s tongue looks oversized, and he’s biting the sides. Surgical intervention can narrow the tongue. I’d say wait a few years to see if he grows into that tongue. If not, surgery will avoid future painful lesions.
