I don't think it's fake. To those who say that there isn't enough blood, remember he's in some sort of hospital, so there has to be some length of time between whatever the fuck happened to him and to get there.

Plus, look at his chest/shoulders absolutely splattered in blood. And he's in a hospital so they are most likely constantly cleaning up the wound and area around it.

Possible fake, because his bottom half jaw would be dangling in front of his chest or so. My thoughts on reality.
