I think that is the camera angle. In that picture, his body, (and not because of the penis), looks more male, but, the face still looks female to me.

Ok i got it right this time. Its actually a man this time and yes, the best pics cant be shown here.

The scrotum has been cut and reshaped to create an artificial vagina, while the testicles are moved into the labia. This means that the penis is still fully functional in being able to become erect and ejaculate, but the urethra no longer carries urine along the penis (TLDR: They can still shoot cum but have to pee like a girl).
