This can be a standard treatment for several injuries. A friend of mine was hit in the face with cricket ball and sustained a compressed fracture of the cheek bone. His scalp was cut by the surgeon similar to the photo so he could push his hand down to the cheek bone and pull it back out without leaving a facial scar. The scar was in the hair line so you could not see. Btw he was in and out of hospital the same day.
