A 60-year-old male presented with a major crush injury to his right foot. He was referred to plastic surgery. His forefoot was badly crushed. He underwent a forefoot amputation. his hind foot looked fine with less degloving. He was reluctant for amputation, the rest of the foot was salvageable. A free tissue transfer was planned but the recipient's vessels were not good and he was subjected to negative pressure wound therapy, a method of drawing out fluid and infection from a wound to help it heal. His foot condition improved and underwent skin grafting. After the healing process, the patient can walk without footwear and also when wearing a stuffed sports shoe. The patient and his relatives were satisfied with the outcome and thanked the team.
