I assume from the statement that he refused surgery in the past that he lives in a location where medical care is available, so let's eliminate unavailability of proper care as a reason.

I think the only remaining possibilities fall in the mental health category. It could be an extreme case of being in denial. And there are some mental health conditions (such as learned helplessness, among others) that can cause people to not take obvious steps to care for themselves even when those steps are available to them.

Whatever it is, it's sad to see. I'd hate to think what the prognosis would be for quality-of-life if that much of your neck dies and rots. Will he retain a functioning neck? What about damage to blood vessels & nerves? Not good.
