Live .223 Caliber Rifle Cartridge Stuck in Urethra

A 21-year-old man with no prior medical issues inserted a live .223 caliber rifle cartridge (a common AR-15 round) into his urethra for sexual pleasure.

He had a history of the practice but called this his widest attempt. After 24 hours of unsuccessful self-removal attempts, he sought care.

The cartridge was found lodged in the distal (front) urethra, with the primer (ignition) end visible near the meatus (urethral opening).

No major symptoms like pain, bleeding, or inability to urinate, but the live round posed an explosion risk.

Bedside attempts (e.g., stone retrieval basket) failed due to the cartridge's size and shape.

Surgeons opted for open removal in the operating room to minimize accidental discharge.

They made a 2 cm ventral (underside) incision over the palpable cartridge, dissected carefully (no electrocautery to avoid sparks), and extracted it intact.

The urethra was repaired in layers over a sound and catheter. Follow-up examination showed no leaks or damage.

The team treated it as live ammunition throughout as centerfire cartridges (like this .223) have a primer in the base that fires when struck centrally—grasping the sides was safer than the tail.

Rimfire types (e.g., .22) require even more caution at the edges. A discharge in the urethra could cause shrapnel, tissue damage, or projectile launch, endangering the patient and staff.

Non-medical urethral sounding (also called "cock-stuffing" or recreational urethral insertion) involves intentionally placing objects into the urethra for sexual stimulation or gratification.

While medical urethral sounding uses sterile, smooth dilators to treat strictures or aid procedures, the non-medical version often employs improvised or commercial items and carries significant health risks.

This practice appears in medical literature primarily through case reports due to its rarity, stigma, and potential for severe complications.Literature

Urethral sounding is uncommon but documented, especially among men who have sex with men (MSM).

One study of over 2,000 MSM found that about 10.7% had tried it at least once, with participants showing higher rates of sexually transmitted infections (STIs) and high-risk sexual behaviors compared to non-practitioners.

Women engage in it far less often as in one 15-year review of 27 cases at a single institution, only one involved a female patient.

That’s actually a “blank” with no projectile bullet tip. It’s scrunched closed at the top so all it has is powder and there’s very little chance this bullet would “fire” due to the primer being on the bottom needing to be struck before it fired.

Wow... where to begin. Technically the urethra is a 'bottomless' orifice, but you've really gotta TRY to lose something in there. Good job, I guess? Too bad it was a blank, if he'd managed yet botched at-home removal he might've shot himself in the face. I've done that, a couple times...

1. Why did he use a blank cartridge? There is no projectile in that round.

Maybe he gave the tip to whoever helped him get it up there?

I know this can be a religous issue, but personally I'm not in favour of circumcising bullets. I think it makes them less effective for their intended purpose.

Actually circumcised bullets can be more effective upon impact with a target than uncircumcised bullets.
