Perhaps I am not looking at it from the correct angle (like I have another choice), but it looks like his left leg meat got squooze'd up into his abdomen, causing it to do an insta-bloat.

Anyone?

No that’s his butt cheek.
His left leg is across the body with his hips twisted, and his knee is up under his back with foot extended out. (Toes peeking out just to the right of him)
His right leg is just out from under him

And a big ole’ rrrrrip up the crotch. It’s a girl!!!
