A 55 yr old male patient was referred for an upper GI endoscopy for symptoms of GERD, hoarseness of voice and foreign body sensation in the throat for the last 2 months. A detailed general physical examination was unremarkable. An UGI endoscopy was attempted but he was not able to swallow the scope. As the scope was positioned on the larynx, they saw a live leech which was coming in and out of the vocal cords. They tried to catch it with the biopsy forceps but it was too slippery. The posterior sucker was firmly attached to the tracheal wall. Patient was referred to the ENT surgeon who removed it under general anaesthesia in the OR

It is a preventable. NEVER DRINK WATER DIRECTLY FROM THE STREAM.
