The picture of the young guy in the blue shirt is special effects makeup. (1) The burns have that "fresh" look, they should be scars at that point. (2) If the burns were that bad all down his chest he would not in any way have any semblance of a nipple or the ability to put a ring there.

I believe it's a before and after picture. The weird nose could be a pedicle tube graft. The tube pedicle was developed simultaneously yet independently by Sir Harold Gillies at the Queen’s Hospital Sidcup and ophthalmic surgeon Vladimir Petrovich Filatov in Odessa, Russia between 1916 and 1917.
