You're an idiot our chromosomes do determine our gender. There are mishaps in nature where chromosomes maybe mixed up which is your example. The majority of sex change operations are legit woman or men with the correct chromosomes. However because you dont feel like a man or woman on inside doesnt mean genetically you are . So no matter if the make a penis for them or not still down to the smallest cell they are still a female. I think in most cases before surgery is performed genetics testing should be done. Also a very thorough mental evaluation needs to be done.
