He is dead...a thoracotomy is a last-ditch effort to save someone's life. He had a wound to the lung and heart...probably a stab wound, since there is an incised wound above the thoracotomy. The arrow points to the wound in the heart.

Looks like a type of thoracotomy to access the heart/lungs/certain vessels and can't see his ribs 'cause just the intercostal muscles between the ribs are cut
or he just got his ass kicked
