He is dead...a thoracotomy is a last-ditch effort to save someone's life. He had a wound to the lung and heart...probably a stab wound, since there is an incised wound above the thoracotomy. The arrow points to the wound in the heart.

Looks real - cut looks like it's from a fine saw of some kind or possibly a filet knife. Either way they're pretty screwed because he burst the pleural sack and deflated a lung.
