Loss of nose and upper lip in car accident

This 17-year-old man was taken to the emergency room 2 hours after an amputation of his nose and upper lip as he was thrown through the windshield in a car accident.

The amputee nose and upper lip were wrapped in gauze and put on hold at the scene of the accident. The upper lip was amputated in continuity with the nose resulting in a 90 % defect of the upper lip.The amputated nose and upper lip segment was immediately taken to the operating room for examination.

Microsurgical brackets were used to mark the upper labial arteries (1,0 mm diameter) bilaterally and the left dorsal nose artery (0,5 mm). Another dissection was carried out cranially by the glabellar wound to move further away from the injury zone.

A suitable 0,8 mm vein was found cranial 3,5 cm. A 4 cm interposition vein transplant was taken from the media side of the right foot. When venous anastomoses were completed, a heparin bolus with 5.000 units was administered intravenously.

A good venous drain has been reached and maintained. The patient was kept on bed rest for the first 3 days with his head up in a warm room (26-28° C).
His diet and activity have been gradually increased. Postoperatively, no complications occurred.

During his 18-month follow-up, the patient showed an excellent consistency of the nose airways and was very satisfied with his aesthetics.
