Cloacal bladder exstrophy, seen here on a newborn infant. This looks painful... and smelly... and fragile... not to mention life-threatening if it gets infected or (God forbid) punctured!

This condition has to be repaired for the child to survive long-term. Usually they do an operation right after birth to protect the internal organs, and then several more reconstructive surgeries a little later.
