must be a pleural empyema, collection of pus in the fluid surrounding the lungs.

but i have no idea why it looks like theres a balloon of skin expanding on his back everytime he seems to bear down...

I think you're right- you're very smart!!! It appears to be his lung---or a small alien hiding in there. The skin is very bruised . Maybe broken rib resulting in severe infection(?). lung could be coming through that way. I sure hop the infection is isolated and no pus is flowing around inside the center and upper part of his body.
