Let's sleuth it out -that's nice,clean flesh but the borders are irregular.He could have had some kind of drug resistant infection (a boil or bite) that had to be cleaned out with medication (debrided medically) Or-he got drunk and passed out without feeding his cats..

maybe the joker took him and rachel to some warehouses and attached them to some kind of gasoline, then gave batman the wrong address and when batman came to save him, just as the building exploded the gasoline lit up; he fell in the gasoline while trying to escape, so he burned his face but he refused to have surgery because he was very angry with the corrupt police force selling him out, and rachel dying because of that and all.
