Degloving Injuries: Ring Avulsion
Ring avulsion is an injury caused by a sudden force pulling a ring from a finger, often when a wedding band catches on machinery or a protruding object. Injuries range from a soft-tissue cut to complete degloving or amputation. The Urbaniak classification separates injuries by circulation and tissue loss. Class I injuries retain adequate circulation and are treated with soft-tissue repair. Class II injuries have inadequate circulation and require urgent restoration of blood flow. Class III injuries involve complete degloving or amputation. Although revision amputation was historically favored for class III injuries, later literature supports replantation when key joint and tendon structures remain intact. Reported survival after class III replantation was 78%, compared with 80-90% for finger replantation generally. |
