Not using a nail or screw, but I saw this type of thing before on a video; supposedly it feels really good to have a sounding rod inserted and taken out of the urethra, for males and females, and some are using it as a means of pleasuring themselves. You have to be really careful though - if the rod is not sterilized and any bacteria gets in there it could lead to a massive infection. It's interesting, but not my cup of tea.
