A 43-year-old schizophrenia sufferer from João Pessoa, Paraíba state, gouged out his own eyeballs during a psychotic break.

According to his brother, the man had returned from church when voices started to urge him to remove his own eyes. Upon discovering what had happened, the brother placed the eyeballs in a bag of ice and transported his sibling to hospital. The man was reported as being in a stable condition, but the damage to his eyes was irreversible.

I can top that. A coworker got a call about a client, a schizophrenic, just released from the mental hospital. Upon going to the home shared by the recipient and his mother, they discovered him in the kitchen with a big butcher knife on the table and him seated in a chair by said table. He had hacked off his penis and testicles, which were lying on the table beside the knife. He told her "It's okay. They will grow back". Paramedics were called to take him to the hospital due to bleeding and then he was readmitted to the psych hospital. I don't think they grew back.
