I don't think they do for this kind of operation.

My niece recently had a brain operation where some of her brain was removed. She had a huge scar running from the top of her head to the bottom of her ear. Some Skull still has to be removed to get to the brain. I imagine that video is zoomed in an the opening isn't actually that big but I'm not an expert.
