The muscle tissue has lost color. Looks like he was injecting himself in that area and it became infected to the point of rupture if you see the upper portion of the wound. the lower looks almost like it had to be opened surgically to allow the wound to be cleaned and prevent swelling from ripping more tissue. I know several roid heads. They're all asking for it when they start sticking themselves with engineered garbage. Just another form of body modification.
