That's not fat removal or a tumour. When you are obese and lose a lot of weight quickly your skin stays stretched and just hangs there. Looking at the scars in the 1st photo he probably had a gastric bypass to shrink his stomach and lost the weight. The operation in the photos is basically a tummy tuck to get rid of the excess loose skin out of the way.
