diagnosis of locally advanced trabecular, nodular cbc, currently with involvement of the left hemiface and extension to the contralateral orbit, maxillary sinus, lateral nasal wall and exposure of the nasal cavity -- history of multiple basal cell carcinomas starting at age 15, requiring approximately 18 interventions, pectum excavatum and first degree family history (sister) with basal cell carcinoma at age 21. he received vismodegib from february 2022 to december 2022 with clinical and imaging tumor progression, so it was considered primary resistance was suspended and then received radiotherapy 6 mv 3 gy until total dose 30 gy in ptv 1 (mass + margin) until february 2023 with palliative indication, but was suspended due to ocular toxicity. since march 2023 immunotherapy with nivolumab was started, completing 15 cycles with adequate tolerance to date and adequate clinical evolution, consider the disease clinically stable, no control tac report is provided. he has pending screening for keynote-158 study, and new images requested by oncology. consider that the patient benefits from continuing management with nivolumab until the initially proposed cycles are completed, after finishing nivolumab, consolidation with radiotherapy will be evaluated, a control appointment is ordered in 3 months. treatment to date -18 surgeries (last 2018) rla with left eye and soft tissue loss -vismodegib formulated feb.22(not dispensed) fi aug/22. received 6 cycles, until 01/24/23, discontinued due to tumor progression (primary resistance) with extension into right orbit. - radiotherapy imrt 6 mv 3 gy up to total dose 30 gy of in ptv 1 (mass + margin) (palliative radiotherapy not completed due to ocular toxicity) - nivolumab |

one of the worst cases of bcc i've seen i think. ![]() probably a combination of light skin type, sun UV light exposure and hereditary factors contributed to this. Especially when you live in the red/purple areas of the world. |
