In a situation like that, it may be that the brain actually decide to develop outside the skull, or maybe an extremely bad case of conjoined twins that didn't get very far? Provided the child's brain is where it's supposed to be, I think surgery may be an option but it would be extensive, and who knows what other problems may be hidden.
