This is a perfect example of stage 4 decubitus ulcers due to prolonged immobilization.

The one large hole in the middle lines up with the sacrum/ coccyx area, and the two on the sides line up with the ischium (the bottom "wings" of the pelvis if you can visualize it being butterfly shaped). Patients like this tend to not survive.

I’d bet a million dollars it’s bed sores.... I see them all the time in the nursing homes or at home bed bound people.
