I don't know in this case, but my guess is that isn't actually as cringeworthy painful as it looks, because when you have deep cuts that have gone through the nerve ends you don't feel much pain. I know when I was a kid and fell over onto a lump of broken glass I cut my knee open literally down to the bone, I saw and poked around my knee bone and never forget thinking how pure white and shiny it looked. It didn't hurt at all, but having the bugger stitched hurt like hell.
