This looks like Klinefelter's Syndrome, which involves a degree of gynecomastia. This is caused by a trisomy of sex chromosomes if the form of XXY. These are males that carry an extra set of female genes, generally resulting in breast formation, underdeveloped testes and mental retardation can also occur.
