If my memory serves me well, I believe that she or he is married to a woman who is also having a sex change. The service must have been quite bizarrely worded ..... "Do you take this woman to be your lawful wedded husband ... ".

This looks like Klinefelter's Syndrome, which involves a degree of gynecomastia. This is caused by a trisomy of sex chromosomes if the form of XXY. These are males that carry an extra set of female genes, generally resulting in breast formation, underdeveloped testes and mental retardation can also occur.
