This woman was assaulted and shot in the head with a shotgun by her former partner. On the left side of the occipital region of the head straddling the midline and involving the upper neck was a skin defect measuring approximately 7 cm × 15 cm (2.7" x 5.9"). Extending from this defect were splits that radiated to the left upper ramus of the mandible, to the left occipitotemporal region and up into the parietal region. Around the periphery of the wound on its right-hand margin and inferior margin was a rim of abrasion and including oblique and tangential pellet-type lacerations.

There was scalloping of the wound margin. The anterior face was unaffected. Circular pellet defects were also present. There was one outlying pellet defect lateral to the right margin of the large defect. In addition, there was a separate defect immediately behind the lobe and lower portion of the left ear. Torn and fragmented muscle and bone were present in the base of both the large defect and the smaller defect behind the ear.

A shotgun pellet was present within the hair adjacent to the wound. Hair was demonstrated to enter the larger wound and exit the smaller wound behind the ear. This close-range shotgun wound involved a path that traversed the back of the head. Various components of the wound comprised both individual and complex pellet entrances and exits together with wounds from the fragmented portions of bone. Cause of Death: Shotgun wound to the back of the head.

Postmortem CT showed significant fragmentation of the left parietal, temporal, and inferior occipital bones. There were no fractures of the frontal bone or the right side of the cranium. There was complete fragmentation of the left posterior and middle cranial fossae with fractures extending into the right occipital bone as well. There were linear fractures which extended to involve both orbital plates. There was a significant number of shot fragments distributed throughout the cranium and superior neck. The cervical spine was, however, unaffected.
