I'm gonna guess that spoon is because he was getting a fix ready. I think i see a syringe lying next to the spoon. Also, i don't think those are recent scars, i think they might be lines due to lividity because he may have died on his side. I sure as fuck am no expert, and layman would even be generous, but i'm a curious sort who likes to know, and i enjoy trying to figure shit out.
