Above that guys foot, and what I take is his shoulder, you can see part of a hydraulic ram. It's that round rusty thing with bolts in it, and a piece of shiny metal coming out of it.

His hand is holding onto a hydraulic line, probabley trying to pull himself out when the machine started up. Those things don't move real fast, it's not like death was instantaneous.
The victim new that machine was gonna crush him.

Either way industrial accidents suck for the victim. My biggest worry is spilling coffee on myself or the keyboard.

What an awful way to go, knowing that you're about to get slowly squashed to a pulp and not being able to get free
