He probably did not want to become a woman. There is a whole subculture of males who amputate both penis and testicles willingly. When it becomes a compulsion it is sometimes listed under "body dysmorphic disorder" where the person does not feel "whole" or "right" until the offending appendages are removed.
Yes, that illness is really terrible!

Yes, that illness is really terrible!
There are no known treatments for it either, but who are we to tell them they cannot be what they want to be? It may be a compulsion but studies have shown that once the person is who they desired to be the symptoms either drastically lesson or totally disappear.
