This has actually been determined to be the work of a serial killer, along with the other four similar cases Simone has posted. The police indicated that in all cases he drugged them into unconsciousness before strangling them, so it may have taken less physical effort and strength than it would on a struggling victim. But yes, there would still have had to be a good deal of prolonged force in order to get her tongue to pop out like that.
