I think you are right - it's foam, not his tongue. I have no idea why - they are not drowned, which is often the reason for foam.

The new set of images shows that there are in fact water under the bridge, so he could have drowned.

I don't know if the bus landed on the roof and is turned around, but I guess it did. If not killed by the impact, the passengers would easily drown then. On the other hand - the driver saved his life so far, so at least he didn't drown.

Possibly, they were injured when the van hit the bottom and then drowned.
