Yes I've seen this pic before. Cannot remember the story with it, but it was a good couple of years ago when I first saw it. I vaguely remember it being something to do with a train.

Has to be shopped, the skull has even thin thickness right down through the jaw, heads are not made that way, the brain cavity stops at the bottom of the eyes and the jaw is an entirely different structure and it is massive compared to the top of the skull. There is no way that could be the interior of a real skull.
