Man Falls Into Tank of Diesel
A worker fell into a diesel tank while cleaning it, he wore only a mask, and not the company issue breathing apparatus he was supplied with. He was taken to the morgue. Turns out he had no experience in that type of work at all

Looks like he and the company are both responsible, these days you usually see a large hose pumping fresh air into the hole when they do underground work.  |

That's confined space entry and there are numerous steps to do before safely breaking the plane of the manway.
