Hmm. His "fail-safe" failed. I guess he thought that by looping his right arm through one end of the noose, when he lost consciousness the end would slip off his arm letting him fall to the floor taking the pressure off his neck. Guess he didn't count on his elbow actually flexing and holding the noose in place. I would say "Live and learn" but, well, he failed at the "live" bit.
