I'd say it is an untreated cooking fire burn, though this is the most severe example , if it is that. That or birth defect of course.

Looks like exposure to Radiation in my opinion. If it's from WW1 or WW2 could be a survivng victom of a nuclear explosion and the radiation possibly did that.
