either attacked from behind or axed when unconcious. i dont imagine someone would stand and wait for an axe to fall in those areas when an axe wielder moves not too fast and telepaths the evil intent a lot of time before it hits.

cheek wound and shoulder done in a single stroke. angle of cut not really plausible when attacked standing since the axe would have to come from way way above head to have that effect.
