Maybe a fall from a big height. Blood comes out his ear and runs down his face. His upward cheek (his chocolate side :D ) is nearly intact, only a litte deformated because of the crushed skull underneath, the eye and most of his face poped out.

Or maybe he got hit by a vehicle and splashed face front on the pavement (or sidewalk if you prefer) after a long flight. Would be a possibility too.
