What kind of accident involves two loops of rope around neck? Children were playing hangman?

They were swinging on the rope and using it to climb a tree if I recall. Somehow it got entangled around her neck in the process. The brother tried to rescue her once she fell and was actually choking out, but wasn't strong enough to lift her quickly. He eventually got her down and loosened the rope, but he had no idea how to revive her and went for help. By the time he got back, she was dead (may have been even before he got her down) and already being eaten by ants, as evidenced in the photo.

I believe the girl in the photos is 11 or 12 and the brother was 14.
