How the fuck did this happen?
The 2 solid steel bars/posts/ whatever look to have rolled off the truck before he could get the lifting straps around them for the forklift.One of the straps is over his shoulder and the other still on the truck.He may have jumped off the back and got crushed as they fell??
Either that or the load fell out of other straps/ lifting gear??
